Job Description
JOB SUMMARY
The Care Manager at North Mississippi Health Services plays a critical role in coordinating and delivering direct patient care to an assigned caseload in the Home Health/Hospice department. They conduct thorough assessments, develop individualized care plans, and ensure accurate documentation while collaborating with the healthcare team to optimize patient outcomes. As a manager of process, this role optimizes operations within a designated department or area, ensuring productivity and effective management of processes.
JOB FUNCTIONS
Nursing Care:
- Has the direct responsibility of caring for an assigned caseload of patients.
- Responsibilities include comprehensive assessment defined by COPs.
- Develop and implement appropriate revisions to individualized plans of care.
- Deliver highest standard of care specific to patient condition/need.
- Responsible for appropriate and timely documentation of care and supervisory visits according to CMS and agency guidelines, policies and procedures.
- Additional responsibilities include: utilizing community resources as needed, maintaining an open and effective line of communication with the total health care community.
- Performs a comprehensive assessment of patient with each visit.
- Must maintain accurate and up to date patient clinical records; accurate accounting of supply usage; accurate daily visit record of patients, time and mileage.
Communication: - Communicates and coordinates patient care with patient/CG and all members of the health care team in various settings: inpatient, outpatient, MD clinics, Home care and Hospice.
- Information for authorizations for care according to various payer time frames.
Education: - Involved with Education / PI Regional Manager in development and implementation of educational material and programs for health care team, patients, families/caregivers, and community.
- Assists with educating clinical staff by case conferencing, joint visits, and structured educational programs.
- Pt / CG education of disease management and skilled care in home environment.
QUALIFICATIONS
Education - Associate's Degree Graduate of an accredited school of nursing. Required
- Bachelor's Degree In Nursing. Preferred
Licenses and Certifications - RN - Registered Nurse - State Licensure and/or Compact State Licensure
- BCLS - Basic Life Support
- Cardiopulmonary Resuscitation (CPR)
Work Experience - 1-3 years
Knowledge Skills and Abilities - Proficient computer skills / system connectivity skills preferred
- Must demonstrate knowledge of the principles of growth and development over the life span and skills to provide care to neonatal, pediatric, adolescent, adult, and geriatric patients
- Attends & participates in clinical conferences, staff meetings, and educational activities
- Communicates with physicians and other health care team members to assure quality patient care
- Participates in after hours, weekend, and holiday on-call rotation
- Serves as a preceptor and mentor for new and existing staff
- Demonstrates initiative to obtain more than minimal expectation for continuing education
- Must possess the ability to maintain a good working relationship with the patient, caregiver and medical community
- Interacts daily with Home Health / Hospice staff, area physicians, vendors and auxiliary resource persons
- Projects a positive image of NMMC Home Health / Hospice to patients, families, caregivers, and community
- Supports the patient and caregiver(s) while in the home without unnecessarily interfering with the household routine
